Insurance
I am an in-network provider for the following insurance companies:

For all other insurances, I would be considered an out-of-network provider. If I am not a provider with your insurance you may choose to file for out-of-network reimbursement and your insurance company may pay for services or a percentage of services. I will cooperate with the insurance company and provide any clinical reports that are required; however, you will be responsible for finding out what is required by contacting them directly.

Some people prefer and choose to use a self-pay option due to their coverage and/or desire for healthcare privacy. Clients who do this have an additional level of confidentiality and control over their own treatment because treatment decisions are made between myself and the client without input from an insurance company.

To make the most educated decision for your treatment, I encourage you to call your health insurance provider prior to our first appointment to inquire about the specifics your mental health benefits. Here are some questions to ask in order to ensure you fully understand your coverage:

Do I need a referral or pre-approval for mental health treatment?

Are there any limitations to my mental health coverage (i.e. visit limits, dollar limits)?

Is there an annual deductible I must pay before the plan pays?

How much will I pay for services after I meet my deductible?

Is there a list of providers that I must see to have coverage?

Payment: All self-pay and out of network session fees are due at the time of service. Cash, check or major credit cards are accepted for payment.

Good Faith Estimate:
Under the law, health care providers need to give clients who don’t have insurance or who are not using insurance an estimate of the expected charges for medical services, including psychotherapy services. You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency healthcare services, including psychotherapy services. You can ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule a service, or at any time during treatment. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate. 60 Minute Intake Evaluation: $180

55 Minute Individual Therapy Session: $160

45 Minute Individual Therapy Session: $140
